DIVEBUMZ STANDARD SERVICE AGREEMENT AND AKNOWLEGEMENT

You understand and agree to the serviced being rendered for annual service

for your SCUBA equipment. You agree to these services provided and will be held financially

responsible for all labor charges and parts required. Equipment should be inspected and tested by the customer to ensure you are satisfied with its function and operation prior to taking possession of the equipment. Unforeseen replacement parts over $20 require prior customer authorization. All regulators incur a $5 Christolube © surcharge for each set. Regulators requiring extra

cleaning due to "Excessive Corrosion" will incur a $15 per stage charge, at the discretion of

the service technician. Upon request, customers will be given the opportunity to view photos of the "excessive corrosion". 3 attempts to notify customers upon completion of services provided. Any equipment left with us more than 30 days after 3rd attempt, become the lawful property of DiveBumz, LLC in accordance with the applicable FL statute.

"Annual Service" is a broad term used in the maintenance of your SCUBA equipment. Annual service should be done each year according to equipment manufacturers. This means either inspection or Overhaul/Rebuild. These services are conducted in accordance with manufacturer recommendations for equipment as outlined in the manufacturer's official Repair Manuals. Any equipment that cannot be repaired will be returned to the customer disassembled.

1st stage- Disassembly, component inspection, cleaning, reassembly with new parts

supplied by the manufacturer, tuning and adjustment.

2nd Stage/Octo- Disassembly, component inspection, cleaning, flush hoses, reassembly with new parts supplied by the manufacturer, tuning and adjustment.

Console- Disassembly, component inspection, cleaning, o-rings replaced, reassembly.

Low Pressure Inflater Hose- Inspect & flush. Remove/replace o-rings & Schrader valve

Power Inflator- Disassembly, component inspection, cleaning, reassembly with new parts

supplied by the manufacturer.

BCD- Leak check, Over Pressure Valve cleaning, Snaps/buckles/velcro inspected and

cleaned, flush & clean internal bladder, clean/rinse BCD exterior.

Computer- Inspect exterior for damage, remove and replace battery, O-ring, and battery

retainer. Computers will be pressure checked and ops checked in a pressure pot.

A $30 per week storage fee will be charged after 14 days for any equipment left at

DiveBumz, LLC.
